Merit Mark Submission Form

Please complete the form below as soon as possible at the end of the season - preferably before October 31. The completed form will be automatically e-mailed to the Commander and Bridge Officers. You will also receive a copy of the completed form via e-Mail (your confirmation of a completed submission).

Note: To qualify for a current-year merit mark, list only the activities performed during the merit-mark year which runs from October 16 of the previous year through October 15 of this year.  This form is submitted once per year (usually in the fall after the boating season comes to and end).  Please reference Squadron communications around the deadline published each autumn.
